SVMC Community Health Needs Assessment 2021

Every three years SVHC conducts The Community Health Needs Assessment to develop a broad assessment of priority needs in our service area. Input from the community is vital to being able to assist the best way possible. Please consider joining the forums and pass this information along to friends, colleagues, and family in our service area.

OVERVIEW: The Community Health Needs Assessment is a triennial process in which data and community input are combined to give a broad assessment of the priority health needs in our Health Service Area. This assessment guides the development of an implementation plan, which outlines the steps SVMC will take to address the identified priority health needs. Every year, we give an update about progress we have made toward addressing these needs. Previous assessments, implementation plans, and updates can all be found in the footer of the home page of svhealthcare.org. The current iteration of this assessment process will look a little different than in the past. We are making a concentrated effort to hear from all corners of our communities, either through one of our scheduled forums, or via a survey (link to be published and distributed soon). The more opinions and information we gather, the more closely our assessment and implementation plan will align with the true needs of our community.

Eastern NY: 3/30
Northern Berkshire: 4/1
Bennington & Southshire: 4/6
